Official Rules for the “Perfect Smile Giveaway”

1. Sponsor

This giveaway is sponsored by [Lazenby Dental, located in Destin, Florida] (hereafter referred to as the “Sponsor”).

2. Eligibility

• The “Perfect Smile Giveaway” is open to legal residents of [state or region where the giveaway is being promoted], who are 18 years of age or older.
• Employees of Lazenby Dental and their immediate family members are not eligible to enter.
• Only one entry per person is permitted.
• Void where prohibited by law.

3. Agreement to Rules
By entering, participants agree to be fully bound by these rules, and they represent and warrant that they meet the eligibility requirements.

4. Giveaway Period
The giveaway starts on Nov 12th, 2024 and ends on Dec 23, 2024. All entries must be submitted by 11:59 PM on the end date.

5. How to Enter

• To enter, participants must [describe the entry method, such as filling out website form explaining why they’re nominated person deserves the smile makeover, etc.].
• Incomplete or inaccurate entries will be disqualified.
• No purchase is necessary to enter or win.

6. Selection of Winner

• The winner will be selected by Dr Lazenby on or around Dec 23, 2024.
• Odds of winning depend on the number of eligible entries received.
• The winner will be notified via [email, phone, etc.] within 14 of the selection.
• The winner has 30 from the time of notification to claim the prize; otherwise, an alternate winner may be selected.

7. Prize

• The prize includes up to $40,000 worth of dental services from Lazenby Dental.
• The actual dental work included in the prize will be determined by Lazenby Dental in consultation with the winner to achieve the best possible result within the prize value.
• The prize is not transferable, redeemable for cash, or exchangeable for any other prize.
• Any costs exceeding $40,000, if agreed upon, must be paid by the winner.

8. Conditions and Restrictions

• The winner must undergo a dental examination and consultation to determine eligibility for specific treatments.
• Sponsor reserves the right to disqualify the winner if the examination reveals health or other issues that prevent them from safely receiving the planned treatments.
• The prize must be redeemed by Dec 23, 2025

9. Publicity
By accepting the prize, the winner grants Lazenby Dental the right to use their name, likeness, and story for advertising and promotional purposes without additional compensation, unless prohibited by law.

10. Limitation of Liability
By entering, participants agree to release and hold harmless the Sponsor and its employees from any and all liability arising from participation in the giveaway or acceptance, use, or misuse of the prize.

11. Privacy
Information collected from participants will be subject to the Sponsor’s Privacy Policy and will not be shared or sold to third parties.

12. Disputes
This giveaway is governed by the laws of Florida, without respect to conflict of law doctrines. As a condition of participating, entrants agree that any and all disputes that cannot be resolved between the parties shall be resolved individually, without resort to any form of class action, exclusively before a court located in Okaloosa County, Florida..

13. Additional Terms
Sponsor reserves the right, in its sole discretion, to cancel, terminate, modify or suspend the giveaway should any virus, bug, unauthorized human intervention, or other causes beyond the Sponsor’s control corrupt or affect the administration, security, fairness, or proper conduct of the giveaway.
